On the Day of Your Visit: When you arrive at our clinic, you will be greeted by one of our receptionists, who will review your Authorization Form to determine the reason for your visit.
Next, if you are a new patient in our clinic, you will be asked to complete some paperwork that will be used to start your medical record. We’ll also ask for a photo ID. You may briefly wait in our waiting area until you are called into one of our treatment rooms. After your visit, you will be given a Work Status Report to take back to your supervisor. If you need to come back to the clinic for a follow-up, the nurse practitioner will schedule a return appointment.

After Your Visit: If you would like Our clinic will send a copy of your Work Status Report to your employer. The report does not include any medical information – just your work status.
If you are referred to another healthcare provider, then one of our clinicians will work with that provider’s office to set up your initial appointment. We will let you know the date and time of the appointment, get you registered, and let you know where to report for the appointment.
